Victim? Causer?

Every time when we read news paper or watching news reported about accident, what is the first thing in our mind?

1) Why so careless?
2) Blame the person that cause the accident?

Indeed, most of the accidents occured because of careless mistake. But have you ever thought that the accident do also can happen because of victims own action?
